I am writing to inform you that Angela Duke, FNP has accepted a position with another practice and will soon leave Perisseia Primary Care & Wellness. We are appreciative of Angela for the excellent care that she provided to our patients and wish her well in all her future endeavors. Her last day at Perisseia will be October 29.
As part of our unwavering dedication to providing exceptional and comprehensive Primary Care services, I am excited to announce that Family Nurse Practitioner, Linda Peabody, will be joining the Perisseia team as of October 18, 2021.
Linda is a seasoned family nurse practitioner who comes to us from another primary care practice in Gwinnett. She began her nursing career as an ICU RN and worked for many years in the Emergency Room at Gwinnett Medical Center. We are pleased to have someone with her knowledge and experience and feel that her compassion and friendly demeanor will make her a great asset to our team.
Perisseia Primary Care remains fully committed to providing a wide range of primary care services, and any care and treatment you received from Angela will continue to be available to you at Perisseia. Should you wish to continue with your current treatment plan, please schedule an appointment with Linda or one of our other dedicated providers at Perisseia. All your records will remain intact upon Angela’s departure and your care will continue, uninterrupted. If you have questions about your current or future treatment needs or would like to schedule an appointment, please do not hesitate to reach out to us at 770-682-2024.
If you wish to continue your care with Angela, please fax a signed medical records release to us at 770-682-2034. A copy will be sent and all original records will be retained by Perisseia.
